Male pseudohermaphroditism and Os clitoris in a miniature poodle with recurrent urinary tract infections

Zusammenfassung
Der beschriebene Fall handelt von einem 14 Monate alten unkastrierten Zwergpudel, der infolge rezidivierender Strangurie und einer vermuteten Intersexualität vorgestellt worden ist. In der klinischen Untersuchung wurden eine Vaginitis und eine über die Vulva hinausragende ca. 1,5 cm breite und 3,5 cm lange Klitoris detektiert. Röntgenologisch wurde zusätzlich zu der Klitorishypertrophie ein Klitorisknochen (Os clitoris) nachgewiesen. Nach der Behandlung einer nachgewiesenen bakteriellen Harnwegsinfektion wurden als weiterführende Diagnostik eine Computertomografie des Abdomens, eine Ausscheidungsurografie der unteren Harnwege sowie eine Probelaparotomie mit einhergehender Gonadohysterektomie durchgeführt. Um zukünftige rezidivierende Entzündungen des Urogenitaltraktes zu verhindern, erfolgten eine Klitoridektomie und die Entfernung des Klitorisknochens. Die anschließende pathohistologische Untersuchung der Gonaden hat den Verdacht auf das Vorliegen eines männlichen Pseudohermaphroditismus (MPH) bestätigt.
Summary
Presented was a 14-month-old female miniature poodle due to a history of recurring stranguria and the suspicion of a disorder of sexual development (DSD). The general examination showed a clitoral hypertrophy, approximately 1,5 cm wide and 3,5 cm long, and a chronic vaginitis due to the enlarged and exposed clitoris. During the following investigations, a bacterial urinary tract infection was diagnosed and radiologically an os clitoris could be detected. After antibacterial treatment, a CT examination and an excretory urography were carried out as well as a clitorectomy with a urethral transposition and a gonadohysterectomy. During these examinations, an inguinal testicle on the right side, a hypoplastic uterus and a small testicle-like node at the end of the left uterine horn were detected. The following pathohistological examination confirmed the suspicion of a male pseudohermaphrodite.
