Zusammenfassung
Osteosyntheseplatten sind bewährte Implantate in derFrakturbehandlung langer Röhrenknochen. Im Laufe derJahre haben sich verschiedene Ausführungen auf dem Marktetabliert. Eine häufig verwendete Platte ist die DynamicCompression Plate (DCP), bei welcher allerdings bisher nureine limitierte Auswahl an Plattengrößen und -stärken zurVerfügung stand. So waren für die Kleintiermedizin die1,0 mm starke Platte für 2,0-mm-Schrauben und die 2,5 mmstarke Platte für 2,7-mm-Schrauben erhältlich. Während diekleine DCP bei stärkerer Belastung sehr instabil reagiert,ist die große DCP in der chirurgischen Anwendung häufigunhandlich. Dies führte dazu, dass neue Zwischengrößenentwickelt wurden. Für die Anwendung bei kleinen Tierensteht nun zusätzlich die DC-Platte für 2,0-mm-Schrauben miteiner Stärke von 2,0 mm sowie die 2,3 mm starke DC-Plattefür 2,4-mm-Schrauben zur Verfügung.Im Rahmen einer Festigkeitsanalyse wurden die vier obenbenannten Platten verglichen und in einem axialen Biegeversuchdie maximale Axialkraft bestimmt. Die Ergebnissezeigten ein eindeutiges Verhalten zwischen maximalerLast, Plattengröße und -stärke. Je stärker und größer diePlatte war, desto höher lag die maximale Last ohne Versagensmechanismus.Die DCP für 2,4-mm-Schrauben wiesim Vergleich zu ihrer Größe das beste Ergebnis auf. Aberauch die verstärkte 2,0-mm-Platte zeigte eine wesentlichhöhere Belastbarkeit als der 1,0 mm schwächere Vorgänger.Die Ergebnisse belegen eine sehr gute Anwendbarkeit dervier Platten bei Kleintieren und ermöglichen eine optimaleAuswahl für den behandelnden Veterinärmediziner.
Summary
Strength analysis of the new 2.0-mm and 2.4-mm intermediateDCP sizes with conventional 2.0-mm and 2.7-mm DCP boneplatesOsteosynthesis plates are accepted implants for longbone fracture repair. Along with several other designs, thedynamic compression plate (DCP) is widely used in theveterinary field. There are only a limited number of platesizes available in small animal surgery. These include the1.0-mm-thick DCP for 2.0-mm screws and the 2.5-mm-thickDCP for 2.7-mm screws. While the smaller DCP for 2.0-mmscrews is often too weak, the larger DCP for 2.7-mm screwsis difficult to handle in the surgical setting; therefore, intermediatesizes of DCP were designed. For the treatment ofsmall animals, additional plates with a thickness of 2.0 mmfor the 2.0-mm screws have been developed as well asplates with a thickness of 2.3 mm for the 2.4-mm screws.In this study, the axial bending of the four aforementionedplates were compared and the maximum axial load wasdetermined.
The results show a distinct relationship between maximumload, size and strength of the plates: the larger andstronger the plate, the higher the maximum load withoutoccurrence of failure. In comparison to its size, the DCP for2.4-mm screws showed the best results. The reinforced 2.0-mm plate showed a significant increase in maximum loadcompared to the 1.0-mm-thick conventional 2.0-mm plate.These results suggest a good applicability of all four platesin small animal surgery, thereby providing the veterinarysurgeon with additional choices.
