Treatment of jaw fractures in young dogs with first dentition and fixation of small rostral fragments

Zusammenfassung
Nerven- und Gefäßkanäle im Kieferknochen, die Anlagen der bleibenden Zähne, der weiche Knochen, das Wachstum und das Vorhandensein der Milchzähne machen die Versorgung von Kieferfrakturen bei Welpen sowie kleiner rostraler Fragmente bei Junghunden kompliziert. Mit Plattenosteosynthesen oder Fixateur externe wird zwar eine gute Fixation erreicht, es werden jedoch regelmäßig wichtige Strukturen verletzt. Milchzähne eignen sich häufig nicht zur interdentalen Schienung. Die Arbeit beschreibt eine neue, minimalinvasive, gewebeschonende Fixationsmethode: Mittels eines Korbes aus kunststoffverstärktem Osteosynthesedraht, der um das rostrale Frakturfragment geformt wird, und durch die caudale Fixierung am Os zygomaticum bzw. am Proc. coronoideus werden weder die Anlagen der bleibenden Zähne noch Gefäße und Nerven verletzt. Dennoch wird eine sehr gute Stabilisierung auch kleiner Fragmente mit dem Ergebnis einer guten funktionellen Ausheilung erreicht.
Summary
Treatment of jaw fractures in young dogs and fixation of small fragments is complicated by nerves, vessels, deciduous teeth growing within fractured bone, smooth bones and growth. Fracture treatment using bone plates or external fixation is rigid but injures these structures in most cases. This paper describes a new, minimal invasive fixation method that takes care of these structures: use of a cage of surgical wire and methacrylate formed around the rostral fragment and caudally fixed within zygomatic bone or coronoid process of the mandible injures neither growing teeth nor vessels or nerves. This construction achieves good stabilization even of small fragments. Functional results are excellent.
