Zusammenfassung
Die Plazentitis gehört zu den häufigsten Ursachen eines Spätabortes oder einer Frühgeburt bei der Stute. In der Regel wird diese Entzündung der Plazenta durch eine aufsteigende Infektion mit Bakterien verursacht. Zu den klinischen Symptomen zählen unter anderem eine verfrühte Euteranbildung, prämature Laktation und vaginaler Ausfluss. Die klinische Verdachtsdiagnose wird mithilfe einer transrektalen oder transabdominalen sonografischen Untersuchung des Uterus bestätigt. Dabei kann die gemeinsame Dicke von Uterus und Plazenta (CTUP = combined thickness of uterus and placenta) vor allem ventral am Übergang von der Zervix zum Corpus uteri einen guten Anhaltspunkt liefern. Endokrinologische Tests und die Messung von Entzündungsmarkern bieten eine zusätzliche Möglichkeit, insbesondere subklinische Plazentitiden zu diagnostizieren. Da die Mehrzahl der Plazentitiden bakteriell bedingt ist, ist eine antibiotische Therapie mit Breitbandantibiotika unerlässlich. Zusätzlich wird die Anwendung eines Entzündungshemmers empfohlen. Durch Applikation von Gestagenen und Beta2-Sympathomimetika wird versucht, das Auftreten eines Abortes oder einer Frühgeburt zu verhindern. Eine frühzeitige Diagnose und Therapie sind entscheidend für die Entwicklung und das Überleben des Fohlens.
Summary
Placentitis is a common cause of late-term abortion or premature birth of often septicemic foals in mares. In most cases bacteria enter the uterus via the cervix through an ascending vaginal infection. The clinical symptoms include premature udder development (with or without streaming of milk) and vulvar discharge. The clinical diagnosis is confirmed via transrectal or transabdominal ultrasonography. A good indicator for placental inflammation and separation is the measurement of the combined thickness of uterus and placenta (CTUP) cranial and ventral to the cervix. Endocrinological and inflammatory biomarkers provide additional information regarding impending abortion or severity of inflammation, especially in subclinical placentitis. Broad-spectrum antibiotic therapy is an essential step to stop further spreading of the infection. Additionally, the application of an anti-inflammatory drug is key to prevent the production of pro-inflammatory mediators. The treatment with progestins and tocolytics should protect from abortion or premature birth. Early diagnosis and treatment as well as continuous monitoring are essential for the development and survival of the foal.
