Septic peritonitis in small animals: diagnosis and therapy

Zusammenfassung

Die septische Peritonitis ist die häufigste entzündliche Erkrankung des Peritoneums beim Hund (Kirby, 2003). Sie tritt meist durch eine sekundäre mikrobielle Kontamination auf. Ursache, Klinik und Prognose dieser lebensbedrohlichen Situation sind vielfältig und werden durch verschiedene Komponenten beeinflusst. Signalement, Entstehungsgrund, Kontaminationsgrad sowie der individuelle Immunstatus spielen dabei eine wichtige Rolle für die Prognose. Trotz moderner Diagnostik und Behandlungsmethoden ist die Morbidität hoch und die Mortalitätsrate schwankt zwischen 20 und 70 % (Greenfield und Walshaw, 1987; Mueller et al., 2001; Lanz et al., 2001; Costello et al., 2004; Bentley et al., 2007). Die schnelle Erkennung der klinischen und pathophysiologischen Symptome sowie die Wahl der richtigen Therapie sind Voraussetzungen für einen erfolgreichen Ausgang.

Summary

Septic peritonitis is the most common inflammatory disease affecting the peritoneum in dogs (Kirby, 2003). It is mainly secondary to microbial contamination. However, this life-threatening condition can have a wide variety of causes, clinical courses and prognoses. Septic peritonitis can be influenced by many different factors: the signalment, aetiology, degree of contamination and the animal’s individual immune status all play a very important role in determining the prognosis. Despite the availability of modern diagnostic techniques and treatments, septic peritonitis is associated with a high morbidity and the mortality rate ranges from 20 and 70% (Greenfield und Walshaw, 1987; Mueller et al., 2001; Lanz et al., 2001; Costello et al., 2004; Bentley et al., 2007). Successful treatment requires early recognition of the clinical signs and appropriate medical therapy as well as careful surgical planning.
