Canine protein-losing enteropathy

Zusammenfassung
Führen gastrointestinale Erkrankungen zu einem massiven enteralen Eiweißverlust, sind die daraus resultierenden Begleiterscheinungen (Thromboseneigung, Körperhöhlenergüsse, Kachexie etc.) lebensbedrohlich. Signalement, klinische Symptome und Laborwertveränderungen sowie bildgebende Verfahren (Ultraschall, Röntgen, Szintigrafie) unterstützen die Diagnose der Eiweißverlustenteropathie – über gastrointestinale Biopsien wird versucht, eine ätiopathogenetische Diagnose zu stellen. Hunde ohne gastrointestinale Symptome mit einer Hypoalbuminämie, für die andere Ursachen, wie Leberfunktionsstörung und Eiweißverlustnephropathie, ausgeschlossen wurden, sind hochverdächtig für eine Eiweißverlustenteropathie. Schnelle und aggressive Therapie der Komplikationen sowie teilweise lebenslange Behandlung der Grunderkrankung können zu einem guten Therapieansprechen führen, die Prognose bleibt jedoch vorsichtig.
Summary
Excessive enteral protein loss in dogs with marked gastrointestinal disease may result in life-threatening problems such as thrombosis or body cavity effusions. The signalement, clinical presentation, laboratory test results, and diagnostic imaging (radiographs, ultrasound and scintigraphy) may aid in the diagnosis of protein-losing enteropathy – the goal is to obtain aetiopathogenic diagnosis by taking gastrointestinal biopsies. Dogs presenting without gastrointestinal symptoms but with marked hypoalbuminaemia are under the suspicion of having protein-losing enteropathy once severe hepatic dysfunction and protein-losing nephropathy have been ruled out. Quick and aggressive treatment of life-threatening complications, combined with treatment of the underlying disease, if necessary for the rest of the dog’s life, may lead to good response, although prognosis remains guarded.
