Canine atopic dermatitis – diagnosis and therapy

Zusammenfassung

Dieser Artikel gibt eine Übersicht über die Diagnose und Therapie der caninen atopischen Dermatitis. Die Diagnosestellung erfolgt durch Anamnese, Klinik und Ausschluss relevanter Differenzialdiagnosen. Zur Therapie stehen dem Praktiker neben der Allergenvermeidung (z. B. bei durch Futter induzierter atopischer Dermatitis) die allergenspezifische Immuntherapie sowie symptomatische Therapie gegen den Juckreiz zur Verfügung. Glukokortikoide, Ciclosporin und Oclacitinib zeigen sehr hohe Wirksamkeiten. Antihistaminika, Fettsäuren (topisch oder systemisch) sowie juckreizhemmende Shampoos sind weniger wirksam, haben aber auch weniger Potenzial für klinisch relevante Nebenwirkungen.

Summary

This article reviews pathogenesis, clinical features, diagnosis and treatment of canine atopic dermatitis. Initially allergen- specific IgE antibodies and mast cells were in the focus, but recently research on lymphocytes and cytokines produced by those cells as well as the barrier function of the epidermis has been intensified. Canine atopic dermatitis is characterized by erythema and pruritus, predominantly affecting the head, distal limbs and abdomen, secondary lesions such as alopecia, lichenification and crusts as well as secondary infections with bacteria and yeast organisms. Diagnosis is confirmed by history, clinical examination and ruling out relevant differential diagnoses. Therapeutic options are allergen avoidance (e. g. for food-induced atopic dermatitis), allergenspecific immunotherapy and symptomatic antipruritic treatment. Glucocorticoids, cyclosporine and oclacitinib show a high efficacy. Antihistamines, essential fatty acids (topical and systemic) and antipruritic shampoos are less efficacious, but also show less potential for clinically relevant adverse effects.
