Zusammenfassung
Erkrankungen des Pankreas und der Leber erfordern spezifische, auf den Patienten abgestimmte diätetische Maßnahmen. Im Falle einer Pankreatitis spielt häufig eine Fettreduktion eine bedeutende Rolle, allerdings wird dieser Ansatz zunehmend kontrovers diskutiert. Handelt es sich um eine exokrine Pankreasinsuffizienz, ist die Zugabe von Pankreasenzymen unumgänglich. Auch die parenterale oder orale Gabe von Cobalamin sollte aufgrund der verminderten Aufnahme beachtet werden. Erkrankungen der Leber stellen aufgrund der zahlreichen Beteiligung an Synthese-, Umbau- und Speicheraufgaben spezielle diätetische Anforderungen dar. Aufgrund der verminderten Fähigkeit zur Stickstoffmetabolisierung ist vor allem der Proteinquantität und -qualität in der Ration besondere Bedeutung zu schenken. Durch die Gabe von Zusätzen, die einen ansäuernden Effekt im Darmlumen haben, kann hier die systemische Ammoniakbelastung gesenkt werden. Eine wichtige Rolle spielt zudem der Einsatz von antioxidativ wirkenden Substanzen als Radikalfänger.
Summary
Hepatic and pancreatic diseases require specific dietetic approaches which should be adjusted to the individual needs of the patient. In case of pancreatitis, a reduction in total fat content plays a vital role. However, the efficacy of this approach is currently under discussion. In case of exocrine pancreatic insufficiency supplementation with pancreatic enzymes is inevitable. Because of a reduced absorption of cobalamin, the parenteral or oral administration of cobalamin must be assured. Hepatic diseases affect the function of the liver for synthesis, storage and modification. Therefore, specific dietetic restrictions reduce the metabolic load in the liver. Due to the reduced capacity to metabolize nitrogen, the quantity and quality of protein in the diet is very important. Systemic ammonia influx can be reduced by supplements with an acidifying effect in the intestinal lumen. Also antioxidants play an important role in hepatic dietetics as radical scavengers.
