Calcinosis cutis secondary to iatrogenic hypercortisolism – a case report

Zusammenfassung

Eine einjährige männlich-intakte Englische Bulldogge erhielt Prednisolon in immunsuppressiver Dosierung zur Behandlung interdigitaler Pyogranulome und Pruritus über elf Monate. Unter der Therapie entwickelte sich eine hochgradige Calcinosis cutis aufgrund eines iatrogenen Hyperkortisolismus. Mit dem Absetzen der Glukokortikoide und topisch-antiseptischer sowie systemisch-antibiotischer Therapie verbesserten sich die Symptome. Der Fallbericht zeigt schwerwiegende Komplikationen durch den Langzeiteinsatz von Prednisolon und stellt die Wichtigkeit der diagnostischen Aufarbeitung und Behandlung einer möglichen Grundursache dermatologischer Patienten in den Vordergrund.

Summary

A 1-year-old intact male English bulldog had been receiving prednisolone in an immunosuppressive dosage for the treatment of interdigital pyogranulomas and pruritus over a period of eleven months. During the therapy, a high grade calcinosis cutis secondary to iatrogenic hypercortisolism developed. The dog’s signs improved with cessation of the glucocorticoid treatment as well as the topical antiseptic and systemic antibiotic treatments. This case report shows the severe complications that can arise due to the long-term use of prednisolone and emphasises the importance of adequate diagnostic processing and treatment of the root causes of disease in dermatological patients.
